Coding Excisions

For an excision which size do I go by to get the correct code, is it the post op or the excision diameter:
Please Help.
Pre op size 1.1cm by 1.2cm
Post op size 1.4cm by 3.9cm
Excision diameter 1.4cm
Appropriate margin 0.2cm
Final repair length was 3.9cm

Excised diameter is what the CPT code says BUT I would query the physician as it seems to be a typo.
